Summer Soccer Camp

By signing this waiver "I Agree" , you agree to the following terms and conditions:
iProSkills Academy Chicago “we” in the following Terms and Conditions document) provides Youth Soccer Programs such as: Soccer School, Soccer Camps, Soccer Clinics, Soccer Tryouts and other soccer related training, events and entertainment.

Medical Waiver
Recognizing the possibility of injury or illness, and in consideration for the iProSkills Academy Chicago, I consent to my son/daughter participating in programs organized by the iProSkills Academy Chicago. Further, I release, discharge, and otherwise indemnify iProSkills Academy Chicago, its member organizations and sponsors, their employees, associated personnel, and volunteers, including the owner of fields and facilities utilized for the Programs, against any claim by or on behalf of my son/daughter as a result of my son’s/daughter’s participation.

My son/daughter has received a physical examination by a physician and has been found physically capable of participating in the iProSkills Academy Chicago programs. I give my consent to have an athletic trainer and/or doctor of medicine or dentistry provide my son/daughter with medical assistance and/or treatment and agree to be responsible financially for the reasonable cost of each assistance and/or treatment.

I agree that if it appears that my child may have sustained a concussion or head injury that he or she is to be removed from the competition until such time that a trained medical professional can examine them and approve their return to play soccer.

Photo Release
The Participant, and the Parent(s) or Guardian(s), if the Participant is a minor child, grant authorization to the iProSkills Academy to use photographs of the Participant for publicity purposes including Website, social media and print advertising content for a period of 4 years after his registration online.

A full refund, minus a $25 administrative fee, will be issued for campers canceling up to 4 weeks before the start of camp. Within four weeks of the camp start date, no refunds are available; you may switch into a different week of camp. If we don’t meet the minimum registration numbers, we reserve the right to cancel the camp within two weeks of the camp’s start date in which instance you will receive a full refund. There will be no refunds or make up session for cancelation, unattended days or sevre weather conditions. The decision to cancel and/or modify a program including camp location and time depends solely on the iProSkills Academy staff.
